根據臨床應用的不同,生物醫學超聲主要可以分為超聲成像和超聲治療兩大類。超聲成像技術透過接收並處理超聲波經過生物組織傳播後的反射和散射訊號,從而獲取生物組織的超聲影象,為醫學診斷提供依據,主要包括B超、多普勒超聲、超聲諧波微泡造影、彈性波成像、超聲顯微鏡、超聲介入導盲等。經過幾十年的發展,超聲成像診斷技術越來越成熟,目前已經與磁共振成像(MRI: magnetic resonance imaging)、計算機斷層掃描(CT: computed tomography)共同組成三大臨床醫學影像診斷技術。超聲治療技術透過將超聲波能量聚焦在病灶區域,利用聚焦超聲波引起的熱效應、力效應和空化效應等,在不損傷正常組織的條件下,實現對病灶區域的直接治療或者輔助治療,主要包括超聲溶栓、超聲碎石、超聲乳化白內障、高強度聚焦超聲腫瘤消融以及本文將要研究的經顱聚焦超聲等。出於安全性考慮,醫學超聲成像所需的超聲劑量一般都小於

10^{-1} W/cm^2

平均聲強,基本不會對生物組織造成損傷。不過,超聲治療在病灶區域所需的超聲劑量一般在

10^2\sim10^4 W/cm^2

平均聲強,除了對病灶區域產生作用外,還有可能損傷正常的生物組織,而且關於聚焦超聲物理效應的量化及其所引起的生物效應的機理仍舊不夠清楚。因此,全面深入的推動超聲治療研究邁入安全有效的臨床應用依舊任重道遠。

經顱聚焦超聲(tcFUS: transcranial focused ultrasound)是一專案前正在蓬勃發展的面向腦部疾病的無創超聲治療技術。透過外部超聲裝置發射特定中心頻率的超聲波(目前最廣泛採用的是中心頻率在0。2~1。5 MHz之間的超聲波),超聲波在穿過顱骨後聚焦在顱內的目標病灶靶區並在目標病灶靶區產生熱效應、力效應和空化效應等,從而實現對顱內病灶區域的直接或者間接的調控和治療。自1940年代初期,Lynn等人首次將中心頻率為835 kHz的聚焦超聲作用於動物頭顱以來,經顱聚焦超聲技術在微無創治療惡性膠質瘤等顱內腫瘤、特定腦區功能的神經調控、老年痴呆症和帕金森氏病等神經退行性疾病的治療、特發性震顫的緩解和治療、血腦屏障開啟、顱內靶向藥物輸運、顱內血栓溶解、神經病理性疼痛的抑制和治療以及癲癇的控制和治療等方面均取得了一定的效果,具有廣泛的應用前景。不過,因為超聲傳播路徑上顱骨的存在,當前的經顱聚焦超聲除了存在前述聚焦超聲治療技術尚未完全解決的物理效應的量化及其所引起的生物效應的機理問題外,從聲學研究的角度來看,還存在由顱骨引起的兩大主要問題:(1)相比於腦實質、肌肉和脂肪等生物軟組織的聲引數基本可以認為是均勻的,顱骨的聲引數具有較強的非均勻性,從而導致超聲在顱骨中發生強烈的折射和散射並在穿過顱骨後發生傳播路徑的偏轉,進而引起實際焦點與預先設定的顱內目標靶區之間產生強烈偏移或者實際焦域的散焦;(2)顱骨的聲阻抗與其周圍腦實質和面板等生物軟組織的聲阻抗差異大,從而導致超聲在顱骨表面發生強烈的反射,進而引起顱骨表面存在嚴重的超聲能量沉積,加之顱骨對超聲能量的吸收極其強烈,這就極易造成顱骨及其周圍腦實質等生物軟組織因為過熱而被灼傷。顱骨已經成為制約經顱聚焦超聲技術發展和邁向臨床應用的最大障礙。

1990年代以來,超聲相控陣技術的發展為解決顱骨給經顱聚焦超聲帶來的兩大障礙提供了契機和可能性。一方面,為了解決實際焦點與預先設定的顱內目標靶區之間產生強烈偏移或者實際焦域散焦的問題,Fink等人提出的時間反轉法成為目前最常用的和公認的方法。在基本時間反轉法輔助經顱超聲精準聚焦過程中,首先,從目標靶區處的聲源發射的超聲波由超聲相控陣接收。接下來,各個陣元接收到的訊號被反轉並作為發射訊號再從超聲相控陣發射,此時,超聲波將會穿過顱骨準確的聚焦在目標靶區。然而,為了實現無創經顱聚焦超聲治療,實際中是不可能採用侵入性的方式在顱內目標靶區處放置聲源的。Aubry等人提出了結合計算機斷層掃描(CT)的虛擬點源時間反轉法過程:首先,透過顱骨的CT掃描檔案獲取顱骨的非均勻聲引數;隨後,目標靶區處的聲源被替換為虛擬點源;接下來,透過聲場數值模擬來獲得的超聲相控陣各個陣元所接收到的訊號;最後,將數值模擬中各個陣元接收到的訊號反轉並作為發射訊號再從超聲相控陣發射,從而實現全過程的無創經顱超聲精準聚焦於顱內的目標靶區。另一方面,為了儘可能的抑制顱骨及其周圍腦實質和面板等生物軟組織過熱灼傷的問題,目前主要的解決方案包括:(1)將上述基於虛擬點源時間反轉法獲取的各個陣元接收訊號反轉併發射,利用數值模擬對經顱聚焦超聲場和聲致溫度場進行模擬,基於該模擬結果尋找顱骨及其附近生物軟組織中的異常聲能量沉積和溫升區域,在異常聲能量沉積和溫升區域設定與其聲壓幅度相同、相位相反的虛擬點源,在顱內的目標靶區處依舊設定虛擬點源,以期在保證經顱超聲精準聚焦在顱內目標靶區的前提下實現對顱骨及其附近生物軟組織中的異常聲能量沉積的抵消,迭代重複該過程,直到顱骨及其附近生物軟組織中的能量沉積不再引起過度溫升為止,最後,基於最終的模擬結果調整超聲相控陣每個單元的發射超聲幅度和相位來儘可能抑制顱骨及其附近生物軟組織的聲能量沉積,從而儘量減少顱骨生熱;(2)將微泡超聲造影劑注射到血管中,當微泡超聲造影劑到達顱內聚焦超聲區域時,聚焦超聲與微泡超聲造影劑相互作用會產生慣性空化效應,從而增強焦域聲能量,這就意味在焦域聲功率一定的情況下,使用微泡超聲造影劑可以相對減少超聲相控陣的發射功率,進而能夠降低顱骨及其附近生物軟組織的聲能量沉積,相對的減少顱骨生熱;(3)在經顱聚焦超聲實驗中,超聲相控陣和顱骨之間需要耦合劑,一般是水,透過設計一套水迴圈冷卻系統,將顱骨外表面即頭皮的熱量不斷經由水迴圈帶走,從而實現儘可能的減少顱骨外表面和頭皮的生熱;(4)在設計超聲相控陣的時候,透過增加其有效聲發射面積以及增強其陣元排列的無序性等方法來儘可能的增強焦點的聲壓聚焦增益,從而相對的削弱顱骨及其附近生物軟組織的聲能量沉積和發熱。通常,在條件允許的情況下,可能最好需要將這四種方法組合起來使用。

綜上所述,透過經顱聚焦超聲建模模擬,焦點偏移和散焦問題可以得到校正,同時,顱骨及其周圍生物軟組織的過熱灼傷問題也可以得到監測和抑制。由顱骨引起的經顱聚焦超聲的兩大難題的解決均依賴於經顱聚焦超聲建模模擬。因此,如何準確的對經顱聚焦超聲進行建模模擬對於指導經顱聚焦超聲實驗具有重要意義。
